Application & Release

All members and prospective members are required to complete our new Bottom Time Membership Application form (one per family membership or individual membership) AND the new Assumption of Risk/ Waiver of Liability Agreement form (one per each member in family).

Important points:

Please be sure to read the club policy on personal dive insurance and sign at the bottom of the Membership Application form.

Any children under 18 will need both parents signature if applicable.

Each club member (including non-diver family members) must sign and date the Waiver of Liability form.

Signatures on the Waiver of Liability Agreement must be witnessed.

Bring to the next meeting or mail to the address on the form.

A single membership receives 1 free air fill and a family membership receives 3 free air fills kindly donated my the Palmetto Scuba Connection Scuba Shop.
